Inclusion Matters, now helping patients stay hydrated!
Inclusion Matters’ Hospital Discharge service works with patients to design an action plan to help support their social and practical needs after they leave hospital and provides support to put that plan into action.
As part of the service, we are supplying the RCHT Voluntary Services and Hospital Discharge Service with CRCC water bottles. The bottles form part of a ‘Going home bag’, where patients receive a pack with basic essentials to tide them over until they can get to the shops. We hope the bottles, with a marker gauge for how much to drink, will help patients stay hydrated. The packs also contain information on how people can access our support services.
