Cornwall Carers Service
Carer Wellbeing Practitioner

We are recruiting a Carers Wellbeing Practitioner. The postholder will complete statutory carers assessments and develop carer support plans working with individual carers to identify and access appropriate interventions that will enable them to continue and move forward with their caring role.  The postholder will participate in the development, promotion and strengthening of the Carers Service identity. 

Number of hours per week:  28
Salary:  £19,796 for 28 hours week
Type of contract: Fixed term until 30 June 2026
Work base:  Home based

The postholder will complete statutory carers assessments and develop carer support plans working with individual carers to identify and access appropriate interventions that will enable them to continue and move forward with their caring role.  The postholder will participate in the development, promotion and strengthening of the Carers Service identity. 

The postholder will have an understanding of carers needs and issues.  They will have an awareness of The Care Act and child protection and safeguarding procedures, equality and diversity with a commitment to providing excellent customer service and will either hold, be prepared to obtain or be working towards Level 3 qualification in Information, Advice & Guidance.  They will have excellent communication skills and be able to work independently across a range of platforms and databases following processes and recording data accurately in the prescribed format, together with the ability to cope with competing demands and work effectively under pressure.  The postholder will maintain professional boundaries and act at all times as a professional ambassador of the service and organisation.
