30 hours per week
£21,000 per annum (FTE £24,500)
Fixed term contract to 30 September 2025 (with potential for extension)
Home based (with outreach)

We are recruiting a Hospital Discharge Inclusion Officer to our Inclusion Matters Service, working with individuals aged 18+.  This rewarding role requires you to provide person-centred support for individuals during and after leaving in-patient services within Cornwall.  This role involves undertaking holistic assessment, focusing on addressing social inclusion needs to increase individual wellbeing.

You will have experience of working in a frontline support role and be able to communicate effectively and empathetically with individuals accessing the service.  You will work with the individual to explore their barriers, what matters to them, using a holistic approach and ascertain their social inclusion needs to develop and implement action ahead and after discharge from hospital.

You will have ability to develop effective interpersonal relationships with vulnerable individuals, team members and with colleagues across health and social care settings, to coordinate support and provide information, advice and guidance, and to ensure a smooth post-discharge period, particularly for those without support networks or family.

Please note an Enhanced DBS check is required for this role, for which the organisation pays.
